The Salty Dog Camp LLC requires all boarding dogs to be current on Bordetella (Kennel Cough), Rabies and DHPP vaccinations. Leptospirosis and Lyme vaccines are strongly encouraged.

I confirm that I own the dog listed or I was given authority by the owner for taking ownership of the dog.

I assure The Salty Dog Camp LLC that this dog is currently in good health condition.

•

I confirm that my dog has a complete and updated vaccination series.

•

I acknowledge that the dog should have a safety collar and proper identification.

•

I confirm that my dog does not have any fleas or communicable diseases. The Salty Dog Camp LLC will not be held liable for dogs that have fleas or ticks on their body. Flea and tick prevention is the responsibility of the owner.

The Salty Dog Camp LLC will take all reasonable precautions to keep my dog safe and secure. I release The Salty Dog Camp LLC, its employees, and owners from any liabilities but including but not limited to injury, sickness, damage, accident, or death while in the vicinity.

If I am unable to pick up my dog at the booked time, I will give reasonable notice in advance to The Salty Dog Camp LLC. I understand that further charges will be incurred if my dog stays longer than the listed boarding dates.

In the event of illness or injury, staff will make a reasonable effort to contact the owner before seeking medical attention. If my dog needs medical attention, I authorize The Salty Dog Camp LLC to contact my veterinarian and seek care understanding all veterinary expenses will be my financial responsibility.

I understand that payment is due at the time of service. Payment is accepted by cash, credit card, or check. Checks payable to; The Salty Dog Camp LLC.
